Why no Starz channels?

Is the promo available to people billed by a third party? My girlfriend's father's account is billed through Windstream.

I had encore about 3 weeks and it disappeared as mysteriously as it showed up. I'm an annual subscriber.

Was there any specific criteria for getting the "gift"? I don't subscribe to any premium channels and i'm assuming that the more packages you subscribe to, the greater the chances for the "gift".
